The town of Midale invites your proposal for the resurfacing of Main Street within the town of Midale. Main Street consists of approximately 91,200 ft2 of surface area; it is the responsibility of the contractor to verify exact dimensions. The contractor shall furnish all material, equipment and labor to perform the work as specified. Proposals will be accepted until 5:00 P.M. on April 10th , 2018.

Proposals must include:

A detailed description of how the resurfacing will be conducted. This includes, but is not limited to:

how the street will be prepared
how the street will be resurfaced (grade/type of material, thickness of material)
how quality of the resurfacing will be monitored/tested
any sealing that may be required

A total lump sum price, inclusive of all applicable taxes. Pricing shall be valid for a minimum of 90 days from the close of the proposal. Pricing shall include all material, labor, transportation, accommodation and incidentals with a cost breakdown as follows:

price to prepare the street for paving (equipment and labour)
price to pave street with asphalt (equipment and labour)
price for material
any additional surcharges, mobilization charges, or third-party charges

Expected start date with a proposed schedule of activities
Saskatchewan workers compensation letter of clearance
Proof of current liability insurance
A list, if applicable, of all subcontractors that may be used over the course of construction

Make it be known that the contractor shall be responsible for:

procurement and delivery of all materials
cleanup and disposal of any excess/waste materials
ensuring that all personnel are informed and properly trained for the use of the power mobile equipment and are supplied with the necessary personal protective equipment
providing and utilizing proper signage or other means of traffic control during work activities
any surveying and locates if required
ensuring compliance with all provincial and federal regulatory agencies

Make it be known that:

Council reserves the right to reject any and all proposals; incomplete proposals will not be accepted
The lowest priced proposal may not necessarily be accepted.

Proponents are welcomed and encouraged to conduct a site visit and assess the current road conditions. Proponents requiring clarification concerning this Request for Proposal are asked to forward their questions in writing to Linda Dugan at townofmidale@sasktel.net .
