APPENDIX A - PROPOSAL CONTENT INFORMATION
Proponents should, at a minimum, include the following information in their proposals:
1.Lease Provision
1.1 A description of provisions proposed to meet section 5. SHA REQUIREMENTS.
1.2 Confirmation that proponent can commence lease agreement no later than January 31, 2027.
•If the proponent cannot commence provision of services on January 31, 2027 please indicate the earliestalternative date. SHA may consider this alternative date or a phased commencement.
2. Home/Facility Information2.1 Briefly describe your home, its location and the space available for lease. 2.2 Describe how long your organization has been in business. 2.3 Briefly describe your home’s capacity and ability to meet SHA’s requirements described in this EOI. 2.4 Verify that there are no plans/negotiations, in process, that would result in a change in management of the home that would impact the partnership with SHA 3. Contract Matters and Costing Schedule3.1 Form of Agreement appended to this Expression of Interest attached hereto is incorporated into and forms part of this Expression of Interest. The Proponent will be required to verify they agree to the terms and conditions in the Form of Agreement for Services.
